The Western North Carolina Patient Centered Medical Home Project is a three year project funded by The Duke Endowment and The Mission Healthcare Foundation, and managed by Mission Health System. The project facilitates collaboration between existing community resources and a number of Primary Care practices in Western North Carolina, to implement a Patient Centered Medical Home (PCMH) model of care. Emphasis is on practice redesign, care coordination and disease management for high risk patients, and data driven clinical interventions that will optimize health status and reduce the need for unnecessary health services. Support for applying for both NCQA Recognition as a PCMH and BCBSNC BQPP participation will be included in the project.
